Our goal is to provide information, resources, and supports to youth and their families to ensure a successful transition from high school to adult living.

 

 

Secondary Transition is the process of preparing youth for life after high school. Transition planning in Pennsylvania is legally required in the Individualized Education Program (IEP) by age 14; however, thoughtful transition planning can begin at any age. Three main postsecondary goals, in the areas of postsecondary education/training, employment, and independent living,  must be addressed in secondary transition planning.
